We believe that patients should be well informed and involved in their care, therefore we are changing the way your Diabetic annual review is performed.
When it is time for your annual review, we would like you to attend the drop in blood clinic situated at our Leighton Road site, where you will have a blood test to check your blood sugar, kidney function and cholesterol, and a urine test. During this appointment you will also be weighed, your blood pressure and pulse be measured and your feet examined. The Healthcare Assistant will then make an appointment for you with one of the Diabetic nurses.
When your blood test has been reviewed by a doctor or Diabetic nurse the results will be posted to you and you will be able to think about any questions you may wish to ask or things you would like to discuss with the nurse.
At your appointment with the Diabetic nurse she will discuss your results and medication with you and ways that you can manage your Diabetes. If you have any queries that the nurse is unable to answer the Diabetic nurse will have access to a GP with specialist interest in Diabetes in this clinic.
